Columbus River Dragons Liability Waiver
I am aware that any and all activities arising from my participation with the Columbus River Dragons involve inherent risks, dangers & hazards, which can result in serious personal injury or death. I am also aware that hockey rinks contain certain dangers that cause serious injury or death. I herby freely agree to assume and accept all known and unknown risks or injury arising out of participation in any Columbus River Dragons activity. I recognize and acknowledge that risks or my participation in a Columbus River Dragons activity or event can be greatly absolved by choosing not to participate in said event or activity and that my participation is fully optional.
RELEASE AND WAIVER OF CLAIMS AGREEMENT:
For allowing me to participate with the Columbus River Dragons, I agree to the fullest extent permitted by law, as follows:
1.) TO WAIVE ALL CLAIMS that I have or may have against the Columbus River Dragons, the Columbus Civic Center and its owners and affiliates, arising out of participation in a Columbus River Dragons Activity.
2.) TO RELEASE the Columbus River Dragons, Columbus Civic Center and its owners and affiliates from liability for any loss, damage, injury or expense that participant (and participant's parent(s) or legal guardian(s), if applicable) may suffer arising out or participation in an activity with the Columbus River Dragons from any cause whatsoever, including negligence or breach of contract on the part of the Columbus River Dragons in the operation, supervision, design, or maintenance of its activities.
Participation (and participant's parent(s)/guardian(s), if applicable) acknowledge that they have been provided and have read the above paragraphs and have not relied upon and representations of the Columbus River Dragons, that they are fully advised of the potential dangers of Columbus River Dragons on-ice activities and understand these waivers and releases are necessary to allow the continued participation of volunteers for Columbus River Dragons activities. I have read this waiver carefully and accept full responsibility for my participation with the Columbus River Dragons.
